You must read the section yourself, I am just giving you the highlights.
Notwithstanding any other provision of this chapter……whenever a revocation is imposed upon a person for a refusal to submit to a chemical test…and such person has:
(i) within the previous 4 years been twice convicted of any provision of section1192…or
has twice been found to refuse to submit to a chemical test, …..or,
(ii) within the previous 8 years convicted three times of any provision of section 1192…… or has been found
on three separate occasions to have refused to submit to a chemical test, or any combination of the above,
such revocation shall be permanent.
1193.2(b)(12)(b) The revocation is Waivable after 5 years

It is beyond the scope of this book, but be advised that there is now mandatory screening or assessment procedures depending on the charge.
